This form is for Salaried/Exempt employees to submit hours worked per pay period. Salaried workers are paid based on a weekly salary based on an average of 8 hours per day or 40 hours per week. You will not be paid for more or less than 8 hours per day worked.  Entering more or less  than 8 hours is for statistical purposes and for compliance with the Ohio wage law that requires daily work hour records be kept for all exempt and non-exempt employees.

This form also assists us in tracking non-worked days such as vacation, holiday, and sick days, as well as unpaid days taken off. Non-worked paid days go by the standard benefits policies.

Paid Time off depends on the availability to you regardless of what is put on this form. You will only be paid for vacations, holidays, and sick time that you have accrued and that you are eligible to take as of that pay period.
